US Cardiology Volume 9 - Issue 1 - Spring 2012

In this latest issue – Volume 9 Issue 1 – Larry A Allen, MD and John S Rumsfeld, MD contribute an article entitled ‘The Hospital to Home Quality Improvement Initiative’ which was launched in 2009 by the American College of Cardiology and the Institute for Healthcare Improvement. The program focuses on three main concepts: patient understanding of, and access to, medications; routine early follow-up with a healthcare provider following discharge; and comprehension of signs and symptoms that require medical attention, and finally how to seek urgent care in a timely manner. Elsewhere, Todd M Brown, MD and Melody Sherwood, MD review the role of cardiac rehabiliation in heart failure patients.
